Booking Forms are available online as PDFs below:
Teacher Preview Booking Form (PDF)
When completing a Booking Form, Save A Copy of the PDF on your desktop and then type directly into the Form. Completed forms can be emailed, scanned, printed off and posted or faxed.
If you would like a hard copy mailed to you please contact us.

Bookings are processed in order of receipt and an invoice will be issued as confirmation of your booking (reservation). Payment for all school bookings must be made at least one week prior to each chosen event, or your booking will be cancelled and late fees will apply.
Attending the Malthouse Theatre
All tickets for performances will be held at the Box Office, in the foyer of the Malthouse Theatre ready for collection on the day. Please note that latecomers may not be admitted.
For workshops and classes, please meet at the Box Office and wait for Malthouse Theatre staff.

What to expect from a Malthouse Theatre show
We love having you in the theatre here at the Malthouse and we have a commitment to making and presenting great quality, innovative new Australian theatre and reinvigorated, reinterpreted classics. Our work is fresh and the nature of making new work is that things can change right up until opening night. If we feel that the content of a work may inhibit yours or your students’ enjoyment of the experience we will get in touch.
